    AN ACT TO AMEND SECTION 62-5-504, AS AMENDED, CODE OF LAWS OF SOUTH CAROLINA, 1976, RELATING TO DEFINITIONS IN CONNECTION WITH A HEALTH CARE POWER OF ATTORNEY, SO AS TO CONFORM THE SOUTH CAROLINA STATUTORY FORM FOR THE HEALTH CARE POWER OF ATTORNEY EXECUTED ON OR AFTER JANUARY 1, 2007, TO PROVIDE FURTHER FOR A SUCCESSOR AGENT, TO INCLUDE A HIPAA (HEALTH INFORMATION PORTABILITY AND ACCOUNTABILITY ACT OF 1996) AUTHORIZATION, TO CLARIFY DESIGNATION CHOICES IN CONNECTION WITH TUBE FEEDING, AND TO PROVIDE FOR AN OPTIONAL ACKNOWLEDGEMENT BY A NOTARY PUBLIC; AND TO AMEND SECTION 62-7-405, AS AMENDED, RELATING TO THE PURPOSES FOR CREATION OF A CHARITABLE TRUST, SO AS TO DELETE THE REQUIREMENT THAT A CHARITABLE TRUST INSTRUMENT OR REPORT BE FILED WITH THE ATTORNEY GENERAL. - ratified title
